Permission Agreement/ Release

In consideration of having California Foundation for the Performing Arts, its owners, assigns, employees and agents (hereinafter “The Foundation”) provide services to our child, the legal guardian, with full authority to act on behalf of child as the child’s legal guardian, agrees to the following:The legal guardian and child release, waive, discharge and covenant not to sue The Foundation for any loss of damage, and any claim of damage therefore on account of injury to the child or property, while the child is in, upon, or about the premises or any facility of equipment therein.
The legal guardian and child hereby assume full responsibility for and risk of bodily injury or property damage due to the negligence of The Foundation or otherwise while in, about or upon the premises of The Foundation and/or while using the premises or any facilities or equipment hereon and agree to indemnify The Foundation for any and all claims.The legal guardian and child further expressly agree that the foregoing release, waiver and indemnify agreement is intended to be as broad and inclusive as is permitted by law of the State of California and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
The legal guardian and child give permission to The Foundation to photograph the child participating in this event or activity for advertising purposes for The Foundation. It is agreed that compensation will not be received for such use.

  • Students are expected to complete each session paid for. No refund will be given to students who terminate early unless due to an emergency situation.
  • Students will be dropped without notice for unpaid tuition. Any student who attends class with an outstanding balance will be permitted to sit in class but will not receive a lesson until tuition is received.
  • No make ups will be given for lessons missed by the student.
  • Students are to be picked up promptly at the end of class. Students who are picked up 5 minutes late will be charged $5.00 plus $1.00 per minute thereafter. This charge must be paid directly to the instructor by the following lesson.
  • Unless we have a written note on file students will not be allowed to walk home.
The legal guardian has read and voluntarily signed the release and waiver of liability and indemnity agreement, and further agrees that no oral representations, statements of inducement apart from the foregoing written agreement have been made.

Ann Goodman
Ann Goodman Piano

Miss Ann holds an M.A. in Music and has many years of experience teaching kindergarten in public schools. Her extensive teaching experience makes her especially talented at working with children with special needs.

Brian Tandiama
Brian Tandiama Piano & Guitar

Mr. Brian is a graduate of Cal Poly Pomona with a B.A. in Music. Although his specialty is guitar and drums, Mr. Brian is also an excellent piano teacher. Mr. Brian also has extensive experience in performing.

Ronda Costantino
Ronda Voorhees Piano, Voice & Acting

Miss Ronda is the Director of The Academy of Music. She has a B.A. in Music from Cal State Long Beach and a teaching credential from Cal State Fullerton.
